Please fill out this form in order to have NPE PTA fulfill a payment. If you have already made the payment yourself on behalf of NPE PTA please fill out the "Reimbursement Form". This form is if the payment has not yet been made and you are requesting a PTA Check, Debit use, Amazon Purchase, SAMS Purchase, Oriental Trading etc.

Please fill out in order to be reimbursed by the NPE PTA. This form is if you have already made a purchase on behalf of the PTA and need to be reimbursed.
If you are requesting a disbursement from the PTA please fill out the other form. Thank you.
If you need petty cash for an event, please use the disbursement request form and email (or text) the treasurer the number of cash boxes you will need.
